Step 1: Download form & email to or drop off at your doctor office.
Step 2: Ask for the form to be completed by a nurse.
No doctor's appointment needed.

How You Can Help Our School...

· Scrip program - Turn shopping into earnings for our school
· Home & School: Parent & teacher organization for parents of students (preschool - 12th grade).
· Register or sponsor our upcoming Cardinal Classic golf outing.
